We have casual and full-time Registered Nurse positions available working in the Perth Northern and Perth West Coast suburbs.

Clinical in-home services include care plan assessment, medication management, incontinence care, wound care, chronic disease management, complex wound management, palliative care, health and diabetes management Clinical delivery and supervision of non-clinical team members in a community setting Intake processing, clinical care planning and reporting Conducting client care assessments with a focus on Consumer Directed Care Coordinate the provision of quality person-centred care according to the client's needs, service/care plans and available funding according to the client's budget General administration for documentation relating to client care and in alignment with AHPRA regulations and legislation On-call roster Qualifications Qualifications
Qualified Registered Nurse Current AHPRA registration First Aid & CPR certification Australian National Police check Professional Indemnity Insurance Experience in assessing and planning care needs within the community Valid Driver's Licence and reliable, presentable car A smart mobile phone (iPhone/Android) Preferred Skills Preferred Skills
A passion for high-quality client-focused care in a community setting Certificate IV in Training and Assessment - highly desirable Exceptional communication skills Able to work autonomously or within a team environment Excellent time management and organisation skills Commitment to ongoing professional development
