Care Coordinator – Immediate Start (FULL TIME) Company Background: Right at Home is an international leader in the home care industry. We provide care for seniors and adults with disabilities who need assistance to maintain their independence. Position Purpose: This position places a strong emphasis on prioritizing customer satisfaction and involves engaging with families, conducting in-home care assessments, and formulating customized care plans tailored to meet the specific needs of our clients. The role also encompasses supporting administrative personnel in various aspects such as recruitment, training, and overseeing the performance of our caregivers and nurses.
Duties include: Assisting with handling high volume of incoming telephone and internet enquiries. Collaborate with the scheduler to efficiently schedule services for our clients, taking into account the qualifications and availability of care workers. Meeting with clients and assessing their needs. Preparing in-home care assessments and drafting care plans to best meet client requirements. Training and managing the performance of the care team. Maintaining client and care worker compliance and documentation according to government standards. Ensure all documentation is accurately completed in Right at Home databases and software programs. Providing "on-call" services on rotation after hours, on the weekends, and on public holidays. Working Hours: This position is a Monday to Friday position, predominately 8.30am - 4.30pm (can be flexible).
QUALIFICATIONS: Possession of either current AHPRA registration (RN/EN) or a Certificate III/IV in Individual Support. Current Australian National Police check, Working with Children Check. Current First Aid & CPR certificate. Driver's license and your own reliable registered vehicle. Excellent verbal and written communication skills. A smart mobile phone (iPhone / android). Preferred Skills: At least 2 years experience as a scheduler, case manager, care coordinator, or NDIS support coordinator. Solid knowledge in the Disability and/or Aged Care sector(s). Strong time management and organizational skills. Job Type: Full-time
