We're currently looking for a full time Care Coordinator to join our client coordination team servicing our Queensland – Gold Coast Region.
Working days: Monday to Friday 8:30am - 4:30pm.
Across Australia Right at Home provides in-home aged care and disability support services, supporting people to live independently. Our commitment is to improve the quality of life for those we serve.
In this position, you will be responsible for the delivery of high-quality Care Management Services to our Home Care Package clients, including:
Provide service coordination, assessment, care planning and reviews. Determining health and well-being maintenance and promotion strategies aimed at encouraging independence and assisting the client to maintain a quality of life. Ensure all reporting and documentation requirements are met, including completing and maintaining timely, accurate and efficient administrative and communication systems. Provide practical assistance relating to meeting quality and audit requirements. Manage a prescribed caseload and monitor funding budgets. This role will report to the Clinical Care Manager

Diploma - Community Services - Case Manager or similar Diploma within industry with case management experience. Diploma EN (with AHPRA registration) or working towards is desirable, but not essential. Experience in Aged Care, with knowledge of Home Care Packages and Aged Care standards is desirable, but not essential. Familiarity with the Aged Care Act. Ability to write effective and concise care documentation. Experience maintaining computerised customer information systems. Excellent organisational and time management skills and ability to work within limited time frames. Uphold compliance to Policies & Procedures and WHS and the Right at Home culture. National Police Check (Valid within the last 12 months). NDIS Worker Screening Check. Evidence of COVID-19 vaccinations. Current First Aid & CPR. Australian Drivers Licence.
