The Role We are currently seeking applications from experienced RNs with an ability to maximise occupancy in residential care facilities by identifying appropriate potential residential care clients and personally guiding them through the assessment and sales process, until a vacancy occurs in the clients preferred care centre.
Key responsibilities: Review all new applications for Residential Care for the allocated sites. Identify the clients that appear to meet the required ANACC and care parameters for the specific sites. Contact the identified clients and arrange to meet with them and appropriate family members in their home, hospital, or transition care. Complete a preliminary Aged Care Financial Instrument (ANACC) assessment, accessing information from direct assessment and information from family and other health care providers as appropriate. Assuming the preliminary ANACC meets the set threshold, proceed to explain the fees and go through the terms in the residential care contract. Take the potential client and/or the family on a tour of the care facility. Maintain direct contact with the family while waiting for a vacancy in the preferred care centre. Maintain close contact with the allocated care centres, so that there is knowledge of any potential vacancies. When a vacancy occurs, take the family on a tour of the specific room. If the room is accepted, work with the Customer Service Coordinator for Residential Care to have the residential contract signed and the resident admitted as soon as possible, to maximise site occupancy. Build and maintain a good business relationship with Amana Living TCP sites and third party TCP sites to ensure clear communication and availability of new applications for Residential Care. Build and maintain good relationships with key referral groups such as Aged Care Assessment Teams (ACATs) and Social Workers in the hospitals feeding the allocated care centres. This role is offered on a permanent part-time basis working 45.6hrs per fortnight on a fixed term contract until 31st of October 2024. You must be able to commit to all days/hours below to be considered. About You You are a clinical leader capable of using your clinical knowledge and skills to work with the team in pursuit of excellent care outcomes. You are closely aligned to the Quality of Care Principles, and are able to support, educate and mentor the rest of the team.
Your excellent verbal, written and interpersonal skills will be critical to your success in this leadership role, and you will be passionate about staff support, education and mentoring.
Registered with the Nurses' Board of WA as a General Nurse Experience in assessing clinical and general client needs Proven ability to lead and manage a team Knowledge of aged care services to meet individual client needs Dynamic with good clinical understanding Valid Police Clearance no older than 6 months (or ability to obtain at interview stage) NDIS Screen Check (or willingness to obtain at interview stage) Evidence of current Flu and Covid Vaccinations (including Booster if eligible) Ability to pass a pre-employment medical Willingness to attend a paid 2-day (Monday & Tuesday) induction at our Rivervale Training Institute How to apply If you are interested in this role and meet the essential criteria, please click the "apply now" button.
