Residential Aged Care (Rac) Funding Assessor

Working as part of a team across Albany and the surrounding areas, you will enjoy autonomy in carrying out your work, traveling to aged care homes to conduct Residential Aged Care Funding Assessments using the AN-ACC tool.
While direct care isn't part of the day-to-day work, you will use your expert clinical skills and knowledge to conduct interviews with residents and staff and review health records to inform the AN-ACC assessment.

What you bring: 5 years' post-graduate experience in aged care including working with people with dementia.
Relevant tertiary qualifications in Nursing, Physiotherapy or Occupational Therapy with unrestricted AHPRA registration.
Current Influenza vaccination evidence.
Ability to conduct clinical assessments using a range of assessment tools.
Strong organisational skills and the ability to work independently and as part of a team.
A genuine desire to improve the lives of older Australians.
Your own vehicle as travel will be required within your region.
