We are seeking an experienced, enthusiastic and self-motivated Registered Nurse with a genuine passion for working within Aged Care to join our team at Hawthorn Village, Blackmans Bay.
This position is full time, primarily working Monday to Friday.
CHT provide person-centered care, based on individual preferences and choices within our residential care facilities and community care services. We do this from a model of care based on our values of Compassion, Honesty, Respect, Integrity, Services and Truth.

The successful person will demonstrate:
A supportive approach when working with residents and clients with respect to their independence and making informed choices about their care, services and living the life they choose; An ability to lead assessment and care planning in partnership with the resident; Establishing and maintaining effective communication with residents and others that the resident wish to involve in relating to their assessment and care planning; Knowledge of the Aged Care Quality Standards and the accreditation process; Maintain a solid knowledge and contributing to the AN-ACC model; Proven leadership skills and an ability to mentor team members; Adaptability and flexibility including the ability to deal with change; A high level of organisational and prioritisation skills and an ability to multi-task; A commitment to quality continuous improvement; Exceptional written and verbal communication skills; A commitment to Continuing Professional Development (CPD). Essential requirements of the position are:
Bachelor of Nursing qualification; A current registration with Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse; A current First Aid Certificate or willingness to undertake and maintain the equivalent throughout employment; An approved NDIS Worker Screening Clearance or a willingness to undertake one; The provision of an Immunisation History Statement from the Australian Immunisation Register confirming that the applicant has received a minimum of two doses and Booster of the COVID-19 Vaccination; Applications including a covering letter, outlining your skills and experience relative to the position, and a Resume should be forwarded to Amy Bunder People and Culture Manager via the 'apply now' option on SEEK by 9.00am Friday 15 November 2024.
