This is a full time position that is allocated 0.5 FTE to MCU @ Home and 0.5 FTE to referral/consult handling.
MCU @ Home Program ResponsibilitiesAssessing patients for suitability to join the program and discussing with the consultantCompletion of admission and discharge paperworkReviewing patients at home and discussing with the consultantArranging and following up on investigations, medication reviews, scripts, etc.Monitor patients' progress and discuss with the team and consultantArranging telehealth appointments and other appointments as requiredAttending and leading weekly case conferences and family meetingsParticipate in the after-hours and weekend on-call rosterReferral Management ResponsibilitiesReview Rehabilitation referrals in a timely manner and efficiently, discuss with the consultant, and organize for suitable patients to be transferred to the rehabilitation wardContinue to review patients who are not ready for transfer and update the referring team regarding issues identifiedDecline unsuitable patients after discussing with the rehabilitation consultant and update the referring team regarding reasons for declining the referralThe successful applicant will have a current general registration with the Australian Health Practitioner Regulation Agency as a Medical Practitioner and highly effective communication and interpersonal skills, including the ability to relate positively with and appropriately with patients and others.
