Right at Home Perth Northern Suburbs & Perth West Coast provide a range of care services to assist our aging population living independently at home. This Registered Nurse position is on a Part Time (various hours) working in the Perth Northern and Perth West Coast suburbs.
This role is responsible for:
Clinical delivery and supervision of non-clinical team members in a community settingIntake processing, clinical care planning, and reportingConducting client care assessments with a focus on Consumer Directed CareCoordinating the provision of quality person-centred care according to the client's needs, service/care plans, and available funding in accordance with budgetClinical in-home services include care plan assessment, medication management, incontinence care, wound care, chronic disease management, complex wound management, palliative care, health, and diabetes managementGeneral administration for documentation relating to client care and in alignment with AHPRA regulations and legislationOccasional on-call may be requiredQualificationsESSENTIAL Qualified Registered NurseCurrent AHPRA registrationFirst Aid & CPR certificationAustralian National Police checkProfessional Indemnity InsuranceExperience in assessing and planning care needs within the communityValid Driver's Licence and reliable, presentable carA smart mobile phone (iPhone / Android)Preferred SkillsA passion for high-quality client-focused care in a community settingCert IV in Training and Assessment - highly desirableExceptional communication skillsAble to work autonomously or within a team environmentExcellent time management and organisation skillsCommitment to ongoing professional development
