About the Role Grampians Health Horsham is a 20 bed sub-acute facility that provides admitted rehabilitation, geriatric evaluation and management (GEM), transition and palliative care services within a multidisciplinary team and patient focussed environment.
Provide clinical patient care activities in alignment with your educational preparation and level of competence 7-day, 24-hour rotating roster, including day, afternoon, and night shifts.
Permanent full-time, part-time, and casual positions available Provide exceptional patient care Key Responsibilities Deliver individualised nursing care as part of a multi-disciplinary team Liaise and effectively communicate with relevant medical, nursing and allied health team members Promote evidence-based practice that has a problem-solving approach Encourage patients, carers and families to participate in all stages of care When appropriate, supervise and direct clinical practice of junior Registered Nurses, Enrolled Nurses, and undergraduate nursing students and provide feedback Skills & Experience Essential: Registered Nurse with current AHPRA registration Excellent communication and problem-solving skills Knowledge of contemporary nursing theory and practice in relation to the care of sub-acute patients/clients Thorough understanding of quality systems and health services accreditation Desirable: Clinical skills relevant to area Postgraduate / Certificate qualifications relevant to sub-acute care, or currently studying To see a full copy of the Position Description please click here.
