ABOUT THE ROLE You will demonstrate an aptitude for: Delivery of patient-centered care that coordinates individualised care from admission and throughout hospitalisation, incorporating discharge planning in the plan of care; Facilitating communication between the patient, family, medical, nursing, ancillary, and allied health staff; Initiating referrals and utilises available resources for the purpose of providing comprehensive care to patients and families. REQUIREMENTS Current Registration with AHPRA; Well-developed clinical and problem-solving skills in the acute care setting; Be dedicated to providing outstanding patient care; Have demonstrated experience working in an acute hospital surgical unit.
