The Substance Use in Pregnancy & Parenting Services (SUPPS) provides specialised outreach services for substance dependent pregnant women, extending support up to two years postnatally.
Most families referred to SUPPS face multiple, complex challenges.
The service uses a holistic, multidisciplinary approach based on a harm minimisation foundation, to tailor our services to each family's unique needs and goals.
The service is provided in a variety of inpatient and outpatient settings.
Home visits are encouraged and are a key part of service delivery.
The SUPPS RN/RM will: Provide comprehensive assessments, continuous case/care management, therapeutic interventions including motivational interviewing and relapse prevention strategies.Provide assistance in connecting with parenting groups, housing, and relevant groups.Work in collaboration with other LHD services and interagency partners to provide comprehensive support.Minimum Requirements: Current registration with the Nursing and Midwifery Board of Australia.Understanding the principles of substance use disorders and treatment options available including withdrawal management, opioid treatment, and treatment planning for clients with complex psychosocial presentations.Knowledge and demonstrated ability to apply evidence-based practice, quality improvement and person-centred care including Trauma Informed Care principles.Demonstrated ability to use problem-solving skills and a multidisciplinary team approach in the planning, delivery, and coordination of patient care.Demonstrated ability to advocate for and implement individualised person-centred care.Demonstrated understanding of and ability to practice in accordance with legislation, policies, and within the scope of practice of a Registered Nurse / Registered Midwife.Demonstrated effective communication using information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others.Valid unrestricted driver's license for use in NSW/Australia and willingness to work across health sites within the LHD.Need more information?
