Further Details Salary range from $85,000.00 - $95,000.00 PA + superannuationPermanent full-time positionStarting November 2024Located in a small town on the central-western plains of NSWWork alongside a dynamic and supportive multidisciplinary team dedicated to providing high-quality care to the communityOpportunities for career growth within a supportive organisation that values and promotes its employeesKey ResponsibilitiesWork collaboratively within a multidisciplinary team to provide holistic nursing careManage all aspects of Chronic Disease Management, including developing Chronic Disease care plans and conducting Health AssessmentsAssist General Practitioners with clinical procedures and perform diagnostic services, including INR and ECGMaintain accurate and comprehensive patient documentation using Best Practice Appointment and documentation programsEngage in continuous quality improvement initiatives and demonstrate a commitment to ethical work practices and Equal Employment Opportunities (EEO)Participate in community health education and outreach programsSelection CriteriaCredentialsCurrent unrestricted registration with AHPRANational Crime Check within the past 12 monthsCurrent Working with Children's CheckCurrent First Aid and CPR CertificateCurrent 'C' Class Open Driver's License valid in NSWWillingness to obtain a Nurse Immunisation Certificate if not currently heldExperienceA minimum of 2 years of post-graduate experience in a practice settingExperience working in an Aboriginal Community Controlled Health Organisation or within Aboriginal communitiesProven experience in Chronic Disease Management and health assessmentsMust have full working rights for Australia (i.e.
Permanent Resident or Citizen of Australia/New Zealand, or a current, valid visa with no restrictions).
