Join Central Gippsland Health's compassionate team as a Registered Nurse, delivering dedicated palliative care in the Wellington Shire (Part-time, 40 hours per fortnight) About Central Gippsland Health Central Gippsland Health (CGH) is a sub-regional health service and the major provider of health and aged care services in the Wellington Shire.
CGH is located in the City of Sale which is 215 km east of Melbourne.
The City of Sale is an ideal access point for the magnificent Gippsland Lakes, 90 Mile Beach and the Victorian Alps.
CGH provides acute care as part of an integrated service delivery model across Wellington Shire and beyond.
The service provides beds across three campuses (Sale, Maffra and Heyfield).
Acute services include a full-time emergency department, critical care unit, neonatal critical care unit, three operating theatres, day procedure unit, oncology and dialysis services in addition to general medical and sub-acute services including rehabilitation.
CGH is also a leading teaching hospital, affiliated with Monash University's School of Rural Health and provides placements and clinical experience for students.
About Department The Palliative Care team sits within our Community Nursing Department, reporting to the Nurse Unit Manager, Community Nursing.
The Palliative Care team comprises of one full-time equivalent Clinical Nurse Consultant who is supported by the wider palliative care team of registered nurses, Family Support Worker and Volunteer Program.
About the Role The Community Palliative Care Nurse is an important member of the palliative care service team operating in a complex environment characterized by a need to work as part of a multidisciplinary team that supports palliative care service delivery predominantly in the home but also across the care continuum.
You will be required to provide leadership and advocacy to palliative care clients/patients across the care continuum.
You will be required to balance competing priorities and work demands and identify and respond appropriately to critical and urgent clinical needs as well as understanding and operating in a work environment that needs to meet clinical quality and safety guidelines.
Duties Provide effective day to day clinical and emotional support to palliative care clients and their caregivers.Negotiate mutually agreed goals of care and facilitate person-centred decision making and advanced care planning to promote optimal outcomes for individuals, their caregivers and family.Provide complex supportive care using advanced palliative care knowledge and skills in meeting the multiple, complex care needs of individuals with life-limiting illnesses, their caregivers and family, across the continuum of care including bereavement, and in the context of an interdisciplinary approach to care.Manage time and prioritise issues given that work demands can flow from a number of sources and will include deadlines.Demonstrate a high standard of ethical behaviour and professional practice at all times.Qualifications, Skills and Experience AHPRA accredited Registered NurseHave / working towards a post graduate qualification in palliative care or equivalentExperience working in a community nursing service is desirableBenefits Salary packagingEmployee Assistance ProgramSocial ClubApplication Please attach your resume and cover letter to your applicationAll appointments are made subject to the candidate providing a current satisfactory National Police Certificate and obtaining a Working with Children Check.
