The Access Aged Care team provides co-managed primary care services to Aged Care residents across Australia.Your New RoleStep into this exciting, onsite, full-time opportunity where you'll report to the Nursing Regional Manager and become an integral part of our flourishing organisation. This newly created position is focused on improving the health outcomes of our valued customers.In this role, you'll provide clinical management and coordination for our clients and partnering residential aged care facilities. It's perfect for a motivated and passionate individual who thrives in an autonomous role, appreciating the freedom and flexibility offered by working from participating Aged Care facilities. Regular travel to partnering homes for clinical reviews will be required. While the position is advertised for Cairns, occasional travel to other areas may be necessary.As a Registered or Enrolled Nurse, you'll work as part of a multi-disciplinary team to ensure efficient and high-quality service delivery, coordinating the clinical team with the support of the Nursing Regional Manager and administration team. Additionally, you'll contribute to the growth of the resident base through business development efforts.
