About ECHO
ECHO Community Services is a registered charity providing care to the elderly to help them live in their own home with dignity while still maintaining their independence.
We provide in-home services, 7 days per week for Home Care Packages and 5 days per week for CHSP.
Our goal is to provide services at the same time, on the same day and with the same team of Support Workers which means that you know the people who are coming and the time they will arrive.
Having the same support workers providing services, in particular, medication prompts and personal care, maintains the person's dignity and well-being.
You know and can trust the people attending.
There is continuity of care for people who receive our services.
Role:
As a Registered Nurse at ECHO Community Services, you will play a key role in providing direct patient care by closely working with the clinical team.
Your duties will include but not be limited to:
Ensure compliance with aged care standards, regulatory requirements, and organisational policies.
Deliver care in accordance with individualised Care Plans, prioritising safety, respect, and choice.
Adhere to legislation and professional standards, including mandatory reporting and documentation of concerns affecting client well-being.
Manage specialist clinical portfolios, including but not limited to:
Continence and nutrition/weight management
Palliative and wound care
Falls prevention and dementia care
Medication, pain, and infection management
Catheter and stoma care
Conduct clinical assessments and contribute to care planning and reviews.
Foster effective communication and collaboration with clients, families, and other healthcare professionals.
Hours: Monday to Friday, 8:00 am to 4:00 pm, however, you must be willing to be flexible to start outside these hours based on client needs and participate in a flexible roster involving extended hours.
Desired Skills and Experience
Essential:
Current registration with AHPRA as a Registered Nurse.
Previous experience in aged care - residential or community.
Demonstrated nursing care experience in wound care, catheter care, and medication management.
A compassionate, client-focused approach with a commitment to promoting dignity and respect.
Competency:
Excellent clinical assessment, decision-making, and documentation skills.
Strong knowledge of aged care standards, clinical governance, and quality frameworks.
Effective communication and interpersonal skills to build relationships with clients, families, and the care team.
Strong analytical and problem-solving skills, with the ability to organize and prioritize tasks, work effectively under pressure, meet deadlines, and use resources effectively, safely, and efficiently.
Desirable:
Familiarity with the New Aged Care Standards.
Dementia and palliative care experience.
