The position: As a Registered Nurse in Home-Based Nursing (HBN) and Hospital in the Home (HITH), you will provide nursing assessments, clinical care, discharge planning, and care evaluation to patients in their homes.
Under the direction of team leaders, your responsibilities include wound care, palliative care, and post-acute or HITH services.
This role requires adaptability, organisation, and strong clinical skills to ensure patient safety, continuity of care, and emotional support for patients and their caregivers, while promoting patient independence.
