About the Role In this role, you'll provide clinical care, conduct assessments, implement treatment plans, and educate patients.
You'll work closely with the Clinical Navigator and medical team, ensuring seamless care for individuals with complex needs.
This position is integral to the GH at Home program, which focuses on early intervention, prevention, and improving patient outcomes through collaboration and integrated care.
Key Responsibilities Provide information and education to patients, families, and carers, helping them understand treatment options and care plans.
Conduct thorough health assessments for diverse patient cohorts (medical, surgical, pediatric, geriatric).
Monitor and implement care plans in collaboration with the multidisciplinary clinical team, ensuring the best outcomes for patients.
Contribute positively to service improvement, team initiatives, and patient care reviews through case conferencing and team meetings.
Skills & Experience Current registration as a Registered Nurse with AHPRA Experience in acute, sub-acute, and/or community nursing Ability to work effectively in a multidisciplinary team, with strong consultative and collaborative skills Strong decision-making abilities in complex situations and ability to work independently with minimal supervision.
