About the role In collaboration with Wide Bay Hospital and Health Service, this role delivers community-based falls risk assessment and support as part of the wider Fraser Coast Falls Prevention Service Referral Pathway.
Knowledge and skills required Registered Nurse, current registration with AHPRA. Minimum 2 years experience, preferably in a community aged care setting. Demonstrated experience in health assessments and community care, with demonstrated knowledge and understanding of the assessment of complex care, chronic health care, and psychosocial needs. Demonstrated knowledge and experience in community networking in collaboration with other service providers to plan and provide consistent, flexible, and appropriate service responses. Demonstrated understanding and commitment to quality improvement processes, including Occupational Health and Safety and risk management. Excellent written and verbal communication skills, and effective time management skills. Current class C driver licence and NDIS National. Current First Aid and CPR Certificate.
