Hawkesbury District Health Service is a teaching hospital in Windsor NSW, providing acute, community and allied health services for the local community and surrounding districts.
The service provides 24-hour emergency, medical, surgical, diagnostic, maternity, neonatal, paediatric, palliative, intensive and coronary care services.
Our Hawkesbury Community Health team is seeking to recruit Registered Nurses.
The Community Health Care Registered Nurse (RN) provides a range of services to individuals, carers, and families with the aim of maintaining and/or improving their health.
Additionally, you will be responsible for the provision of high quality nursing assessment, planning, implementation, and evaluation of care using evidence based best practice guidelines underpinned by the principles of Primary Health Care.
These strategies are conducted as part of a collaborative partnership with clients, families/carers and other key stakeholders (including the client's GP).
The Community Health Care RN may be required to work weekends and Public Holidays.
This will help ensure care is delivered in a person centred way using a flexible, responsive, wellness and reablement approach.
You will be working in a multidisciplinary team in planning, implementation, assessment and evaluation of patient care.
