About the Role The Registered Nurse – Community Care is the senior nurse responsible for the care of clients in their own home as part of the Home Care Package Program and National Disability Insurance Scheme.
This varied role includes a range of tasks including:
Provision of in-home nursing careUndertaking assessments to support clients accessing services and supportWorking collaboratively to develop and implement individual care plans for complex clientsSupporting CatholicCare's Clinical Governance Framework by providing expert advice to other staff regarding care and incident response for clients with complex needsProactively contribute to the development and implementation of policies and procedures to enhance the services provided to all clientsThe Registered Nurse – Community Care will also provide leadership for the nursing service within the identified region/s and support and oversee other enrolled nurses providing in-home nursing care.
What We're Looking For Bachelor of Nursing DegreeMinimum 5 years of nursing experienceMinimum 3 years community (or aged care) Nursing experience (Desirable)Current C class Driver's license (QLD)In addition to the above, you will need to demonstrate: Ability to assess client needs and develop appropriate plans for interventions and treatment for clientsAbility to develop and provide reports regarding clients needs and conditionsAbility to professionally work with internal and external stakeholders to meet client needsAll offers of employment are subject to the satisfactory completion of relevant screening and background checks, including an Australian Federal Police check, an NDIS disability worker screening check, and attainment of a Blue Card for working with children, prior to commencement.
