The Care Coordinator will be responsible for: Building strong connections with clients and their familiesAssigning staff to clients based on staff qualifications and availability, as well as client preferencesOrganising support for clients with disabilities and monitoring and reporting on their progressReferring clients to agencies that can provide additional helpAssessing client needs and resources for health, welfare, and other facilities and servicesResponding promptly to emails, enquiries, and requests from clients, staff, and managementMaintaining accurate records of incidents and feedback as requiredReorganising work priorities to meet the needs of a fast-paced environmentSupporting all clients to meet their goalsConducting Client Care Assessments and Care PlanningProviding clinical guidance to other team members for client managementDiscussing and promoting Right at Home's services with medical professionals and referral sourcesMinimum Requirements: Current AHPRA registration as a Registered NurseExperience in assessing and planning care needsUnderstanding of the Aged Care and Disability systemsA strong client focus based on their goals, preferences, and needsCurrent National Police Check (or commitment to obtain prior to start)NSW Working with Children Check (or commitment to obtain prior to start)NDIS Worker Screening Check (or commitment to obtain prior to start)NSW Full Drivers LicenceAll candidates must have the right to work in AustraliaOrganisational, administrative, and time management skills #J-18808-Ljbffr
