Seeking a Registered Nurse who strengthens health outcomes through the provision of safe quality, clinically appropriate nursing care in partnership with patients/clients, their families and other health professionals.
The Role: High-level clinical nursing assessment skillsInvolve patients/clients and their families/significant others in identifying achievable goals, focused on re-ablement and maintaining independence in the community settingWork effectively within a multidisciplinary team, contributing to a strong team approach through open communication and a positive supportive approachWith guidance and support from TCP CNC, become a leader in advocating for the frail elderly and promoting safe and person-centred discharge planningThe Team: The Department of Health is committed to improving the health and wellbeing of patients, clients and the Tasmanian community through a sustainable, high quality and safe health system.

As RN TCPC you will provide expert clinical advice and clinical leadership within the transition care setting, supporting the effective assessment, case management, care coordination and implementation of planned, goal-oriented transition care to older people in the community and/or residential aged care setting to enable long term care needs to be met in the most appropriate environment.
Details of appointment: Permanent part time, day worker position, working up to 30 hours per fortnight, as soon as we can onboard you.
*Notwithstanding hours to be negotiated with the successful applicant.
Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.
Salary: $75,902 to $102,908 pro rata per annum.
Our Employer 11.5% superannuation contribution is on top of this amount.
Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023You'll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.
Access to the Grade 4 salary range $96,558 - $102,908 is subject to successful application for progression to Grade 4Post Graduate Allowance of up to 10% based on qualification typeBenefits available to eligible candidates: Professional development and accelerated pathways.Excellent leave benefits: 20 days annual leave (pro rata), 20 days personal leave (pro rata), and study assistance leave if eligible.Fitness Passport – You, and your family can access 40+ fitness facilities across Tasmania for just $13.95 per week per person, or $25.95 for a family.Successful applicants will be required to meet the essential criteria.
Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
