As a District Nurse, you will deliver specialized home nursing care, conducting comprehensive assessments of broader care needs for our diverse community within City of Greater Shepparton and surrounding area.
Responsible for providing a person-centered model of care to consumers and enhancing the health and independence of consumers and their families.
We are looking for a Registered Nurse who has recent experience in community or home nursing care, demonstrating proficiency in working independently, handling varied workloads, and providing interventions primarily in-home settings.
Candidates will need to be able to demonstrate advanced skills in wound care(or working towards) and experience in acute skills required for Hospital in the Home (HITH) .
The successful applicant will have: Current AHPRA registration as a Registered Nurse Strong communication and interpersonal skills Commitment to delivering high-quality, patient-centered clinical care in home environments Effective teamwork capabilities NDIS clearance or willingness to obtain before starting Familiarity with the Commonwealth Home Support Program and its requirements Position Description: HERE If qualifications are a requirement for this position, please attach a copy in support of your application.
