About the Role
We are looking for an Experienced Team Leader to join our team to support a Complex Care Participant based in Gympie.
As a Team Leader your duties include (but are not limited to):
Support and promote the Participant's physical, emotional, and intellectual wellbeing. Develop, implement, and oversee individualized care and support plans for participants with complex medical, behavioral, or psychosocial needs. Utilize advanced communication skills to maintain detailed clinical records, write comprehensive case notes, incident reports, and risk assessments. Engage with Participant's Key Stakeholders and report to Support Facilitator on Participant and Staff Needs. Provide specialized support for Participants with complex needs in all aspects of daily living, including but not restricted to: Personal care (eating, bathing, toileting, bowel therapy, dressing, grooming, getting in and out of bed, and moving about the house). Advanced personal care (e.g., PEG feeding, catheter care, ventilator management). Domestic support; in-home support. Social/community support, communication support, and transportation. Behaviour support and de-escalation, as well as mental health interventions. Pain management, mobility assistance, and manual handling. Complex medication administration. Work within the principles of Social Role Valorisation, Active Support, and Person-Centred Approach. Write and submit reports (including incident reports) regularly to stakeholders in alignment with care plans or goals. Conduct Team meetings internally and with broader RCS Teams (e.g., Operation Management, HR, and Accounts). Maintenance of household inventory and tasks (e.g., monthly cleaning tasks, grocery lists, and medical inventory). Skills & Attributes:
Ability to establish rapport with Staff, Participants, and Stakeholders. Demonstrated experience delivering strong leadership skills, including supporting and providing guidance to Staff in a challenging/complex dynamic environment. Ability to adapt and problem-solve through challenging situations. Ability to work autonomously and take initiative. Strong communication skills; oral and written deliveries, including use of emails (computer literacy is required). Awareness and demonstrated understanding of NDIS Practice Standards, NDIS Code of Conduct, Person-Centred Approach, and Active Support. Qualification Requirements:
Qualification in Disability/Community Services/Nursing (Minimum Certificate III) (Essential). And/or extensive industry experience (Essential). Must have own vehicle and driver's licence (Essential). Current Paid Blue Card & NDIS Worker Screening (or willing to obtain). Current First Aid & CPR Certificate (or willing to obtain). NDIS 'Worker Orientation' Module (or willing to obtain before employment). Full Australian working rights. Smartphone with Internet access. Evidence of current flu vaccination (participant required) (desirable). Clients Support Needs:
Our Participant is looking for a Female Team Leader to join their team in the Gympie Area to provide 1:1 or 2:1 supports. Our Participant requires support with Medication Management and Administration, Complex Personal Care, Hoisting & Transfers. Our Participant faces challenges with Dysphasia and Epilepsy; candidates with experience supporting a person with the abovementioned conditions will be highly regarded.
