About the Service
CREST (Community Re-Entry Services Team) is our corrective services re-entry program in Far North & Central Queensland. The re-entry support officer provides case management support in a range of areas such as access to health services, housing/accommodation applications, employment support, and, but not limited to, drug/alcohol support.
Your opportunity
We currently have an opportunity for a Re-Entry Support Officer (in an Identified role) to join our service on a full-time basis. We are seeking an engaged and passionate candidate to support clients subject to parole orders, particularly as they transition from custody.
This position is an "identified position" and Lives Lived Well considers that being an Aboriginal or Torres Strait Islander person is a genuine occupational requirement for this position under s 25 of the Anti-Discrimination Act 1991 (Qld).
Y our responsibilities will include, but are not limited to:
Support clients to meet their re-entry goals, focusing on individualised plans.
Liaise with, and maintain, stakeholders within the community.
Support clients with complex needs by advocating for their needs and providing case management support.
Complete daily data entries across Queensland Corrective Services (QCS) and Lives Lived Well (LLW) client management systems.
The hourly rate is $41.52 to 44.68 per hour per hour plus super, depending on your skills, knowledge, and experience .
To succeed in this role, there are essentials you'll need:
As this is an Identified position, you must be of Abo riginal and/or Torres Strait Islander background.
Qualifications in relevant human services field OR applicable workforce experience.
Experience in assessment and case management with vulnerable people from diverse backgrounds.
Ability to manage a busy case-load, with excellent time management skills
Highly developed consultation, communication, and interpersonal skills, including a high standard of written and oral communication skills.
Ability to manage a range of administrative tasks to a high standard, including timely and accurate case-noting, and data entry.
Current Drivers Licence.
Be
eligible for QCS Clearance (criminal history check) and ability to provide conflict of interest documentation where applicable.
