The Pharmacy Department is looking for a Senior Pharmacist to join their team.
The pharmacy department at QEII Hospital provides services to a range of specialties including emergency, intensive care, rehabilitation, medical, surgical, palliative care and peri-operative services and includes preparing and supervising the prescribing, distribution, administration and storage of medications throughout the hospital.
Additionally, we provide a number of outpatient clinic services including surgical pre-admission clinic, smoking cessation clinic, chronic pain management and Heart Failure.

About the Role Apply high-level clinical knowledge, skills and leadership applied to single specialties or across clinical areas.Exercise independent clinical judgement in providing accurate patient medication history interviews upon admission, clinical medication reviews, and formulation of pharmaceutical care plans utilising high-level knowledge and clinical skills, including the provision of advice to medical, nursing, pharmacy staff and other health care professionals.As a general specialist in clinical pharmacy practice, apply professional judgement to a complex pharmacy caseload by undertaking reconciliation of medications on discharge, including provision of a Discharge Medication Record where appropriate; communicate with the patient's general practitioner, community pharmacy, continuity of care coordinators, case managers and nursing homes in the discharge planning process.Find out about the role in more detail and how to apply in the attached Role Description.
