The Aged Care Assessment Service is a multi-disciplinary team of nursing and allied health professionals that assesses frail clients aged 65 and older, as well as Indigenous clients aged 50 and over. Our main services include evaluating eligibility for permanent or respite residential aged care, the Home Care Package Program, the Transition Care Programme (a 12-week rehabilitation program following hospital discharge), and Short Term Restorative Care (an 8-week program for addressing functional decline). Once care is approved, the Aged Care Assessor (ACA) connects clients with appropriate community service providers.

About the role:The Occupational Therapist (OT) or Physiotherapist (PT) works across the Aged Care Assessment (ACA) Single Assessment Service (SAS) completing complex / comprehensive assessments in hospital or community settings according to Commonwealth guidelines and the Aged Care Act 1997, bringing an Occupational Therapist or Physiotherapist focus to the multi-disciplinary team and to client care, and referring frail aged clients to appropriate service provision.
About you:The successful applicant must hold a tertiary qualification, or equivalent, in Occupational Therapy/Physiotherapy.Appointment to this position requires proof of qualification as an Occupational Therapist/Physiotherapist and maintain the registration requirements with the Australian Health Practitioner Regulation Agency and the Occupational Therapy/Physiotherapy Registration Board of Queensland, including any necessary endorsements, to be provided prior to commencement of duty.What we offer:Total remuneration value of up to $132,567 is comprised of:
