Job DescriptionQualified Social Worker, Knowsley Metropolitan Borough Council, Adults Hospital Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team.
The team works extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager.
The responsibilities will include:
Support the discharges from the bed-based intermediate care unit working alongside the Reablement/Rapid Response teamHospital experience would be usefulCare ManagementReview Care PackagesAdult PlacementReport writingHome VisitsLiaise with other professionals36 hours per week (flexible by negotiation)2 Month Temporary ContractStart date subject to DBS and reference clearanceBenefitsSuccessful candidates will benefit from:
