We are looking for experienced Aged Care Assessors to join our team.
As an Aged Care Assessor, you will play a vital role in helping older Australians navigate the aged care system, ensuring they receive the right support and services to enhance their quality of life.
This is an excellent opportunity for someone with a strong background in aged care, health, or social services to make a meaningful impact on the lives of older people in the community.
Your opportunity: The Aged Care Assessor within the Wide Bay Aged Care Assessment Team is required to work within a multidisciplinary team and provide clinical assessment services to clients through the use of high-level clinical skills, knowledge, and experience, along with highly effective communication.
The position works within an integrated hospital/community Aged Care Service across the Service Area in order to assess clients to access appropriate support services and/or residential care.This position reports professionally to the relevant Professional Lead and operationally to the Aged Care Assessment Team Leader, Integrated Care, Wide Bay Hospital and Health Service.Several Permanent Full Time (76 hours p.f.)
and Permanent Part Time Positions (hours negotiable) Our offer: You will receive a competitive remuneration package value up to $157,359 p.a., comprising salary between $128,169 - $137,918 p.a., (HP4), or remuneration value up to $131,900 p.a., comprising salary between $107,960 - $115,604 p.a., (Nurse Grade 6 (1)), employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%).
