WSLHD - Provisional Fellow, Perioperative - Anaesthesia Employment Type : Temporary Various Remuneration : - Hours Per Week : 38 Requisition ID : CAM22054 Provisional Fellow in Perioperative Medicine Westmead Hospital caters to a diverse catchment area and provides exposure to a large volume of complex and unique cases. The centre specialises in complex UGIT surgery, Colorectal, Vascular and Urological surgery, complex head and neck surgery with reconstructive procedures. Specialised neurosurgery and cardiothoracic surgery is also offered. Highly complex and acute orthopaedic surgery, major trauma cases, Pancreas Kidney transplant cases are on our emergency lists daily. The Department of Anaesthesia and Perioperative Medicine provides anaesthesia services for approximately 25000 procedures per year. Approximately 5000 patients are assessed preoperatively in the preadmission clinics. In 2023, our perioperative group reviewed over 200 high risk patients, with almost 75% of these patients being outpatient, elective high risk clinic referrals. 23% of these patients went on to receive nonoperative management options after complex shared decision making processes. In this role, the fellow will be attending our High-Risk Perioperative clinic for specific high risk cases only. The high risk clinic will give you opportunity to develop medical optimisation in the complex patient, risk stratification, shared decision making, Multidisciplinary team meetings with our Perioperative Group consultants. You will have the opportunity to be involved in as many of these high-risk cases in theatre as possible, being actively involved in the perioperative journey of these high risk patients. You will be mentored throughout the year by a group of consultants who work regularly together in our multidisciplinary team meetings. In your non-clinical time, you will have rotations through internal medicine training, two to three times a month. You will also be involved in presentation at Grand Rounds at Westmead Hospital, research opportunities within the group. Westmead Hospital is a clinical immersion site for the Diploma of Perioperative Medicine, ANZCA. You will have the opportunity to enrol in this course with the majority of your clinical work contributing to the clinical hours component of the course. All consultants within our Perioperative Group are current supervisors for the course and have been awarded the perioperative diploma (GChPOM) Where you'll be working Westmead Hospital is a key facility of one of Australia's largest major health and medical research precincts and is the principal referral hospital for Western Sydney Local Health District providing district level health services to Parramatta, Holroyd and The Hills Shire LGAs including tertiary and quaternary services to greater Western Sydney, greater metropolitan Sydney, rural NSW and other states. The Westmead Health Precinct currently under development is envisaged to be the largest hospital building project undertaken in Western Sydney and will transform the iconic but ageing Westmead Hospital into an innovative, contemporary, and integrated centre that will continue to deliver high quality healthcare and expand the education, health, and research capability of the area. Westmead Hospital is a leading centre of research, training and specialisation in all fields of Medicine and Dentistry, providing services in Cardiology, Cardiothoracic Surgery, Neurology, Neurosurgery, Kidney and Pancreas transplantation, Cancer Care, Obstetrics and Gynaecology, Neonatal Intensive Care, Trauma, Bone Marrow Transplantation, Critical Care, Aged Care and Rehabilitation, Adult Acute Psychiatry (networked with the co-located Cumberland Hospital) as well as other specialised medical and surgical services. Westmead Hospital is also the principal referral centre for other metropolitan hospitals within the district including Auburn Hospital, Blacktown and Mount Druitt Hospital. Intensive care service in Westmead hospital includes 36 general, and 10 post cardiothoracic surgical ICU beds. General ICU bed numbers have expanded by 50% in early 2020. Cardiothoracic ICU has also recently moved into a newly built ICU pod. Common ICU admissions include complex medical cases such as sepsis, severe respiratory or cardiac failure, complicated bone marrow transplant, and surgical cases including trauma, neurosurgery, vascular, cardiothoracic, ENT, plastic surgery, urology, upper GI, hepatobiliary, and colorectal procedures. What you'll be doing Close engagement with our Perioperative Medicine team including Anaesthetists, Intensivists, and Geriatrician. You will have the opportunity to enrol in the Grand Chapter of Perioperative Medicine with the majority of your clinical work contributing to the clinical hours component of the course. High-Risk Perioperative Clinic two to three times a month, involving High risk patients for Shared decision making, complex medical optimisation planning, risk stratification and Multidisciplinary Team discussion. Opportunity to be involved with the Patient perioperative journey by being the anaesthetic fellow assigned to the High risk cases seen in our High-Risk Perioperative Clinic, and in their postoperative follow up. Opportunity to be involved in complex surgery with members of our perioperative team consultants including major vascular surgery, major UGIT surgery including oesophagectomy and liver resection, major colorectal surgery and neurosurgery. Opportunity to review high risk patients on the emergency list, or be involved in MDT discussions, and be involved in the patients perioperative course. Internal medicine training two to three times a month, through medical specialties to work alongside Consultant Cardiologist, Respiratory Physician, Geriatrician, Nephrologist and Palliative Care Physician. Opportunity for detailed study of perioperative investigation tools such as CPET, echocardiography, sleep studies. During non-clinical time, there will be the opportunity to participate in our current research project, new audits, clinical pathways and teaching. Supervision Arrangements The ANZCA Trainee is supervised by a FANZCA Consultant. ANZCA On Call Consultants on Cardiac, General and Obstetric rosters work on rotating roster and are contactable 24 hours per day, either in person or via telephone after hours. Orientation Hospital Orientation occurs at the beginning of each term for all doctors new to the individual hospital. Departmental orientation for medical officers new to the facility will be conducted at the commencement of each term. Hospital Orientation covers the following: Hospital Tour RSU services (pay, timesheets etc) Medico legal issues Occupational Health & Safety Pharmacy Deteriorating patient and medical emergency calling system Death Certificates and Cremation Certificates Computer access Teaching schedule Infection control Patient Flow Perioperative Medicine Team Orientation will also occur in the first week, with in depth orientation personally delivered by Drs Pendyala & Kim SELECTION CRITERIA: MBBS or equivalent, currently registered or eligible for registration with the Medical Board of Australia in an appropriate registration category Completed four years of accredited anaesthetic training with ANZCA Successfully completed the final part ANZCA examination. Able to work in Anaesthesia under supervision and to work weekends and after-hours in the specialty area and work independently within a supervised complex clinical environment utilizing excellent clinical skills, judgment and expertise Adept ability to work collaboratively within multi-disciplinary teams, including the sharing of appropriate information internally and externally with key persons or services. Adept verbal, written and interpersonal skills, with the capacity to vary communication style in line with the understanding of the needs of diverse audiences Must be able to provide excellent references from current consultant supervisors including the Head of Department. Commitment to participating in training and development opportunities to further develop skills and knowledge in the area of anaesthesia and a demonstrated interest in clinical audit or research.
