Proveda is one of the largest community-led organisations in the Northern Sydney Region, providing independent and impartial information, advice and guidance services to older people, those with dementia, people with disabilities, severe and persistent mental illness as well as those with complex health needs.
We work with people, their families and communities.

Proveda is seeking a dynamic and experienced person to assist people to successfully navigate the care and support system, implement their NDIS & Home Care Plan and to enhance their ability to access the support/s they require.

About the Role
This is a casual role.

Reporting to the Practice Managers, Service Delivery, the Care Coordinator will have experience with Age Care or the NDIS.
The function of the Care Coordinator is to deliver Proveda's model of service to older people, people with disability and people living with mental ill health.

The Care Coordinator facilitates a seamless experience for customers by listening carefully to their enquiry, taking appropriate steps to resolve issues and queries, and following up with customers to ensure that their needs have been met.

Working within a teaming framework, the Care Coordinator provides high quality care navigation services by:
Assisting customers and their families/carers to navigate the NDIS, My Aged Care and the health services system Supporting customers to set and achieve their goals within their plan or package budget Connecting customers with informal, mainstream and funded supports.
Building capacity of customers and their families/carers to manage their own care A Proveda Care Coordinator provides impartial information, advice and guidance to people to implement their NDIS plan, source appropriate providers as well as build their capacity to organise and access their own care choices.

The Care Coordinator will also work with others in the community to ensure the desired and necessary care and support is provided to the client and advocate for systemic changes to occur when a clear and persistent issue arises.

Qualifications in mental health, social work, disability, community services or related discipline Demonstrated comprehensive knowledge of, and experience in service delivery systems including the NDIS, NSW Health or Community NGO service provision Commitment to person centred and recovery approaches to care Excellent communication skills both written and verbal Excellent time management, multi-tasking and prioritisation skills Experience in NDIS A passion for working with people to ensure they get the most out of life Current NDIS worker Screening check & WWCC (or the ability to obtain) Digital Skills including Client record management and Microsoft Suite.
