About the Role: The Office of the Chief Operating Officer (COO) is responsible for leading the delivery of a comprehensive range of health services for the ACT and surrounding district.
It balances the coordination of day-to-day operations while leading strategic development initiatives required for long-term success.
The Integrated Care Program is a key initiative for achieving CHS's Vision, Role, and Values.
In particular, building the connections and establishing the trust required to provide integrated care across the ACT and surrounds.
Patient Navigation is a key element of the Integrated Care Program, with the ongoing delivery and development of the Paediatric Liaison and Navigation Service (PLANS), Liaison and Navigation Service (LaNS) and the Primary Care Pilot.
These services deliver proactive connection and coordination of care with a focus on specific health needs in the community, particularly the needs of individuals with multimorbidity and complex care needs.
The Integrated Care Clinical Project Officer will work closely with the Integrated Care Program Project and Management teams to provide guidance and structure to the ongoing development of patient navigation services.
With limited direction, the role will work with the teams on service development activities including the establishment of processes, development of guidelines, and introduction of initiatives to provide robust foundations for effective patient navigation.
The role will establish or advance service evaluation and improvement activities including the progression of service Key Performance Indicators, Patient Reported Outcome Measures, and Patient Reported Experience Measures.
The position will also establish and undertake service promotion activities, fostering partnerships across CHS and other services.
The occupant of the role will be an inter-disciplinary thinker who can explore problems from different perspectives and communicate ideas and proposals clearly.
Success in this role requires strategic and analytical thinking coupled with strong collaboration and communication skills, particularly the ability to incorporate diverse knowledge and viewpoints while also providing leadership.

Note: This is a temporary position available for a period of 3-6 months with the possibility of extension up to less than 12 months.
