Project Officer - End of Life Care - Indigenous Identified PositionThe End-of-Life Care Project Officer role will support identified clients to navigate access to appropriate local palliative care and end-of-life support services. They will provide advocacy and support to ensure clients receive culturally safe care plans and assessments. The Project Officer will assist clients in understanding their care entitlements, including Advanced Care Planning, contractual arrangements, and costs, and will help ensure community members and their families are well-prepared for End of Life and Palliative Care. This role will liaise with community members and service providers to understand support needed, those already available, and the current/desired cultural practices and protocols associated with the end of life. The role is underpinned by cultural safety and trauma-informed practice.
Qualifications & Experience Aboriginality. Bullinah Aboriginal Health Service Limited considers that being Aboriginal or Torres Strait Islander is a genuine occupational qualification for this position under Section 14 of the Anti-Discrimination Act 1977 (NSW).Qualifications in a health or care-related field or willingness to obtain.Demonstrated experience in a health or care sector role working with Aboriginal and Torres Strait Islander people.Knowledge, Skills, and Capabilities Experience working with Aboriginal and Torres Strait Islander people.Demonstrated project management experience including good record management, data collection, and reporting capabilities.Excellent communication and interpersonal skills, with the ability to build trust with the community.Understanding of person-centered care and trauma-informed practice.Ability to assist clients in accessing services and simplifying service complexities.Sound understanding of Palliative care and advanced care planning.Strong community connections and networks across care providers, mainstream supports, and community sectors.Ability to work autonomously with high initiative and problem-solving skills.Experience working in a team environment that is open, transparent, and focused on person-centred care.Please include in your application: ResumeResponses to Selection Criteria #J-18808-Ljbffr
