The Integration Officer Aged & Palliative Care position supports the strategic objectives of Western NSW Primary Health Network (WNSWPHN) to lead support and strengthen person centred primary health care, by developing and coordinating activities to related to older people's health, aged care and palliative care.
This role will focus on the management, needs assessment, commissioning, implementation, and coordination of WNSW PHNs general practice in aged care (GPAC) response, and related activities. Responsible for the coordination with internal and external stakeholders to ensure considered delivery and monitoring of the response.

Selection Criteria Essential Experience in project management, including creating and managing project timelines.
