The Western NSW Primary Health Network (WNSW PHN) is currently seeking motivated individuals who have a passion for primary healthcare and community wellbeing.
The Integration Officer Aged & Palliative Care position supports the strategic objectives of the WNSW PHN to lead support and strengthen person-centred primary health care, by developing and coordinating activities related to older people's health, aged care and palliative care.
This role will focus on the management, needs assessment, commissioning, implementation, and coordination of WNSW PHN's general practice in aged care (GPAC) response, and related activities.
Responsible for the coordination with internal and external stakeholders to ensure considered delivery and monitoring of the response.

Selection Criteria:
Experience in project management, including creating and managing project timelines.
Experience working in and/or knowledge of the aged care and primary care sectors.
Excellent interpersonal skills, with well-developed written communication skills including writing project reports and management briefings and development of project materials.
Demonstrated understanding of Aboriginal culture including a commitment to cultural awareness and safety.
Demonstrated high-level interpersonal and communication skills and high-level competence in using information communication technology.
Demonstrated organisation and time management skills, with proven ability to deliver high quality outcomes in agreed timeframes.
Capacity to travel within western NSW; hold a current driver's licence.
Qualifications in project management and/or experience in project management.
Experience working in the health, NGO or community services sectors.
Experience in working with Aboriginal people, organisations and communities in view of planning and implementing services and achieving outcomes for Aboriginal people.
