Docklands, AustraliaReference: 6668240Utilise your project management skills to build the capability of Residential Aged Care Homes (RACHs) throughout the north west Melbourne regionFull Time (1.0 FTE), Maximum Term role until 19 May 2025 (Parental Leave cover)Base remuneration starting from approx $70K + superannuation + salary packagingGenerous NFP salary packaging options up to $18,550Docklands office location with option to work some days at homeThe Program Officer, Aged Care Capability Building will be responsible for engaging with and providing support to Residential Aged Care Homes (RACHs) in the north western Melbourne region.
This will involve providing guidance to assist participating RACHs to develop and implement a variety of programs e.g.
The Telehealth training package and General Practice Aged Care Incentive.
The Program Officer will also actively collaborate with other PHN staff and teams on aged care and older adults' initiatives with a focus on improving the health outcomes of older adults in the NWMPHN region.About UsWe are North Western Melbourne Primary Health Network (NWMPHN), Victoria's largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.A typical week may include:Engage with health care providers including, but not limited to, residential aged care providers and staff, pharmacists, allied health professionals and local hospital networksSupport the implementation of enabling tools, resources and guides to build the capability of health care providers.
