Practice Information:This Community Controlled service for Aboriginal and Torres Strait Islander people, works with the local Indigenous community and relevant key stakeholders to increase access to culturally safe, quality, coordinated and comprehensive primary health care services. They focus on healthcare across the life span, including the management and reduction of chronic disease, to address the health and social inequities of Aboriginal and Torres Strait Islander communities of the Fraser Coast Region.Position Description:As thePrimary Health Care Manager you would be responsible for leading the development and delivery of holistic comprehensive primary health care services to the Aboriginal and Torres Strait Islander communities in Hervey Bay and surrounding areas through the Hervey Bay Clinic. The role is responsible for the organisation, coordination, and management of the Hervey Bay Clinic to ensure the delivery of safe, quality and culturally sensitive health careKey Responsibilities:Provide clinical consultation and leadership for the multidisciplinary team ensuring seamless, efficient, effective, and responsive health service delivery.Ensure the clinic and services and operate in compliance with funding agreements agreed on service levels and performance targets.Provide clinical leadership and direction for the multi-disciplinary team in the implementation of the approved Model of Care and effective management of complex Chronic Disease practices, ensuring provision of wrap around services.Lead the Clinic-based team to ensure the provision of high quality, holistic comprehensive primary health care services to the community, in keeping with evidence-based clinical standards in a professional, confidential, and culturally safe manner, which is appropriate for Aboriginal and / or Torres Strait Islander patients of all age ranges and level of health care needs.Lead and embed the implementation of innovative solutions and evidenced based updates, utilising sound change management practices.Identify and manage clinical and corporate risk in all decisions made and in the delivery of outcomes, while also developing effective partnerships with relevant Government and sector network.Ensure all staff undergo appropriate clinical and corporate induction and service orientation, including orientation in the Model of Care with specific job-role induction and training.Key Requirements:Essential:The successful applicant must hold a Bachelor of Nursing DegreeRelevant clinical experience and recency of practice with commitment to evidence-based outcomesDemonstrated ability to comply with the profession's code of ethics and function in accordance with legislation affecting professional practice for maintenance of registration with the Nursing and Midwifery Board of Australia (NMBA) including appropriate level of clinical competence.Current unrestricted APHRA Nursing RegistrationEndorsement for Schedule Medicines for Registered Nurses and Midwives is current.Evidence of effective communication and interpersonal interaction within the clinical teamDesirable:Experience in the provision of safe, quality, holistic, client nursing assessment, care planning, implementation, evaluation of care in the context of Aboriginal and / or Torres Strait Islander peopleHas completed training appropriate to the role including Aboriginal and Torres Islander cultural awareness trainingMembership of appropriate professional peak bodyRelevant Post Graduate qualificationsCurrent ImmunisationNurse Practitioner certificateExperience in Phlebotomy/IV cannulation (or willingness to work towards developing this skill)Experience in Implanon insertion / removal (or willingness to work towards developing this skill)Experience in cervical screening testing (or willingness to work towards developing this skill)Mandatory Qualifications/Professional Registrations and Licences:Qualified Nurse with current unconditional registration with AHPRACurrent C Class Drivers Licence (QLD)Current Blue CardCurrent annual CPR CertificateCurrent First Aid Certificate (every three years)Criminal history checks will be undertaken on the preferred applicant(s).Note: A criminal conviction or charge will not automatically exclude an applicant from consideration for appointment. Rostered 7.6 hours between the hours of 8 am to 6 pm.
