The Federal Government appointed Country SA PHN to enhance the delivery of focused and appropriate health services to the rural communities of South Australia. The goal of the organisation is to improve the efficiency and effectiveness of health services for people, particularly those at risk of poor health outcomes, improve the coordination of health services, and increase access and quality support for people. Job DescriptionThe Role An exciting opportunity has become available within our Primary Care and Digital Support Team.
The Primary Care Support Officer will contribute to the implementation and monitoring of strategies to provide guidance and direct support to the primary care sector, including but not limited to:
Accreditation preparation and maintenanceUtilisation of Medicare Benefits Schedule and Practice Incentive PaymentsRegistration and Meaningful Use of Digital Health technologiesSupport integration across the health sector, specifically connecting the primary health and secondary health care sectors, utilising current and emerging technology and systems to improve provision of services.Continuous Quality Improvement – Plan, Do, Study, Act cycle implementation and monitoringSupport internal and external quality improvement initiatives, identify issues in common, and use this to build innovative solutions.Data extraction for Quality Improvement and PIP QI complianceSupporting the effective implementation and delivery of CSAPHN projects, initiatives and commissioned services that involve primary careDissemination of up-to-date and relevant informationDeliver support and capacity building activities to primary healthcare providers.Provide reports and documentation as required.Support General Practitioners, clinicians and commissioned service providers in the primary care setting, using the stepped care model to select the least intensive level of care, for a person presenting for mental health assistance by using the Initial Assessment and Referral (IAR) tool.Promote and support the effective implementation of quality improvement frameworks and delivery of quality improvement activities.Support uptake and implementation and appropriate use of government initiatives, incentives and systems.Support the collection of quality health data from primary healthcare providers and maintain accurate service and other information in CSAPHN systems regarding primary healthcare providers in the region.Support and inform service design and commissioning across all CSAPHN portfolios by providing insight on primary healthcare provider integration, participation, potential alignment with quality improvement activities.Contribute to the needs analysis and service design activities of CSAPHN by providing insight and input on primary health care needs and primary care provider capability and capacity.Participate in team planning and development activities that contribute to the strategic direction of the organisation.Other duties: Attend and present at meetings, seminars, and forums when required.Collaborate with the Clinical Councils and Community Advisory Committees as required, providing input within the confines of this role.Support the organisation to embed reconciliation initiatives into business and our everyday work in line with our Country SA PHN Reconciliation Action Plan.Country SA PHN offers a great range of benefits to our employees, including:

Desired Skills and ExperienceAbout You Qualifications: A relevant tertiary qualification in a health-related discipline, public administration, business management, health administration or related discipline or equivalent experience in a related field.Experience/Knowledge/Skills/Abilities: Experience in delivery of projects and program outcomes in community, health, or aged care sectors.Demonstrated understanding of primary health care trends and community issues in the South Australian context.Demonstrated ability to build rapport and connections with a wide range of key internal and external stakeholders to ensure best collective outcomes and solutions.Excellent written and verbal communication with strong interpersonal skills and the ability to communicate effectively at all levels.Ability to work with minimal supervision, as part of a team and across business areas, functions, and organisations.Proactive, well organised, with excellent time management and problem-solving skills.Personal Attributes: Demonstrates integrity and is committed to the appropriate and ethical use of data and analytics.Makes tough decisions, sometimes with incomplete information, able to make quick decisions where required.Demonstrates creativity and innovation, generating new ideas.
Actively influences and promotes ideas that improve commissioning and investment of the PHN.Demonstrates teamwork in pursuit of PHN goals.
Encourages resolution of conflict within the group.Knowledge of and commitment to the principles of multiculturalism, equal opportunity and occupational health and safety.Experience with clinical data collection and reporting.Experience in primary healthcare quality improvement activities.Knowledge and understanding of trends in primary healthcare reforms, including State and Federal Government roles in primary healthcare.
