In partnership with general practices and other providers across the Northern Territory, the role drives projects that address issues related to sustainability, safety and quality of primary health care with a focus on improving health care outcomes.The Primary Care Support Coordinator is required to work collaboratively with a range of work units and programs in NT PHN; and to deliver the Primary Care Support Program autonomously, with priority work areas including but not limited to:Data sharing and Continuous Quality Improvement in General Practice.General Practice Accreditation, MyMedicare, Practice Incentive Programs, digital health and other primary care initiatives.Emergency preparedness and management e.g. infectious diseases, cyclone preparedness.Health system integration.Each Primary Care Support Coordinator is also tasked to lead specific portfolios across priority areas, including but not limited to, immunisations, cancer screening, shared care, etc. The allocated portfolio may change based on business needs.If you have the following skills and experience, we'd love to chat:Knowledge or training equivalent to a degree in health care management or other relevant field such as health promotion, public relations, project management; and/or relevant extensive experience in primary health care business management and stakeholder engagement.Demonstrated experience in contributing to continuous quality improvement and working as part of a high-functioning team inspiring change.Demonstrated experience and knowledge of the Australian Primary Health Care System.Highly developed interpersonal skills, with demonstrated expertise in engagement and relationship building and proven ability to build and maintain collaborative relationships across a wide range of internal and external stakeholders in an environment requiring cultural sensitivity.Excellent self-leadership, time-management and organisation skills, with demonstrated ability to work autonomously across multiple projects/programs and prioritise workload effectively with a problem-focused and critical thinking approach.Superior facilitation, oral, public speaking and written communication skills to provide high level and timely advice, build rapport, write and deliver presentations, high-level reports and produce clear and effective documentation.What's in it for You?Working for NT PHN means being a part of an organisation with a bigger picture purpose to improve health care outcomes for Territorians.Enjoy up to $15,899 of salary sacrifice and a generous base salary.Enjoy a vast, fast-paced and challenging work environment where you will work as a part of a dynamic, supportive and collaborative team.Enjoy flexible working arrangements, work life balance and a positive, professional work culture.Receive 6 weeks leave per annum.Enjoy the opportunity to develop your skills, knowledge and career.Have the freedom to apply innovative and creative thinking.Enjoy bringing your dog to work Fridays.Commitment to ReconciliationNT PHN is committed to undertaking all work within a culturally inclusive, sensitive and safe framework that supports Reconciliation.
