ABOUT THE CENTRE OF CLINICAL EXCELLENCE FOR VETERANS AND FIRST RESPONDERS HEALTH: The Centre of Clinical Excellence for Veterans and First Responders Health was founded to support the mental and physical health and wellness of Canada's Veterans & first responders.
This brand new, first-of-its kind Centre of Clinical Excellence (CCE) is located within the Legion Veterans Village in Surrey, BC.
The CCE provides a unique opportunity to be part of serving and supporting our first responders, armed forces, and Veterans.
The CCE focuses on primary and specialist medical care and rehabilitation services, innovative research and health technologies, as well as leading edge treatment for PTSD and mental health.

POSITION SUMMARY: We are in search of a dedicated Primary Care Nurse Practitioner (NP) who will stand at the forefront of our mission to empower individuals to live healthier, happier lives.
This role is a commitment to overseeing the assessment and diagnosis of patients and championing the cause of preventative care, particularly for our cherished Veterans and first responders.
As our NP, you'll ensure the proper monitoring and care for patients dealing with chronic diseases, making every effort to see them thrive.
You will play a pivotal role in ensuring patients receive top-quality, comprehensive care by coordinating internal and external referrals, connecting them to a network of healthcare excellence.

WHAT YOU WILL DO: Provide Primary Care services for Veterans, first responders, and family membersPatient Assessment: Conduct comprehensive health assessments, diagnosis, and treatment planning for patients across a diverse range of healthcare needs, including those within the Veterans and first responder communities.Chronic Disease Management: Manage and support patients with chronic illnesses, including mental health and addiction concerns, with a focus on evidence-based treatment and prevention.Treatment Planning: Develop and implement treatment plans, prescribe medications, order diagnostic tests, and interpret results.Mental Health and Trauma Care: Provide compassionate and empathetic care to individuals dealing with mental health issues, addiction, and trauma, ensuring holistic and patient-centred support.Patient Education: Deliver health promotion and disease prevention education, with an emphasis on empowering patients to actively manage their well-being.Collaboration: Collaborate with a multi-disciplinary healthcare team to ensure comprehensive and coordinated care, with a strong focus on addressing the unique healthcare needs of our target populations.Referrals: Facilitate referrals to specialists and community resources as necessary to ensure access to a broad spectrum of care services.Community Outreach: Actively engage in community outreach programs and initiatives that support our mission and promote overall health and well-being.Clinical Record Keeping: Maintain accurate and up-to-date patient records and documentation in accordance with regulatory standards.REQUIRED EXPERIENCE & QUALIFICATIONS: Master's Degree in NursingA minimum of 3 years of experience as a Nurse PractitionerCurrent licensure and certification as a Nurse Practitioner in British Columbia, with authorization to prescribe medications.Passion for collaborating with experts from various fields to offer multifaceted care that covers lifestyle medicine, counselling, neurorehabilitation, and an array of services designed to support our unique population.Experience caring for individuals dealing with complex health needs, mental health conditions, trauma, and chronic pain.
Veterans and/or first responder work considered an asset.Strong interpersonal and communication skills with a patient-centred approach.Proficient in the use of healthcare technology and electronic health records.A commitment to evidence-based practice and ongoing professional development.Compassion, empathy, and a dedication to providing high-quality, culturally sensitive care. #J-18808-Ljbffr
