We provide programs relating to ageing and disability,health, housing, families, drug and alcohol treatment and supportand more. About the role: This position in our Pathway Home program provides assistanceand support for children returning to their family home. Theprogram is for children and young people who have been inout-of-home care and supports and assists families to ensure astable family environment. The successful applicant will: facilitate assessments and planning processes with peopleaccessing our services by identifying their personal barriersand support needs to develop meaningful ways to overcome thosebarriers; in collaboration with people accessing our services,develop a service plan to guide the intentional support to beprovided, providing regular intentional support based onservice plan and identified support needs, review service plansand support needs regularly and revise plan accordingly; support people accessing our services to find sustainableand meaningful access to natural community resources, servicesand other supports, at the same time as encouraging andsupporting self-advocacy and self-determination; possess relevant tertiary qualifications (Education,Psychology, Social work, Nursing, Counselling and AlliedHealth) or extensive experience and qualifications (Diploma ofCommunity Services) deemed equivalent and/or clearlydemonstrated experience in Community Services. What we are looking for in an applicant: An understanding of the processes people utilise toovercome adversity/distress arising from life challenges. Demonstrated case management skills and ability to workeffectively with clients, family members and service providersto identify solutions. An ability to recognise and understand people'sintrinsic resourcefulness. Excellent communication skills, particularly withindividuals and their families/carers who may be experiencingsignificant distress. Successful applicants must be willing to: provide a current Tasmanian Working with Vulnerable PeopleRegistration (employment/volunteer category) including NDISWorker Screening Check if you are applying for the 1st time orhave renewed your registration since 1 st February2021; present an unsupervised contact with children or vulnerablepeople type National Police Check (no older than 12months); provide an International Police Check if you have residedfor 12 months or more outside of Australia after the age of16; undertake a satisfactory Anglicare assessed medical: provide a current provisional or full Australian DriverLicence.
