Practice Nurse WITH Chronic Disease Management We are looking to recruit an experienced Practice Nurse WITH Chronic Disease Management who is highly motivated, proactive, and positive to join our clinical team.
You will need to be competent in all the usual Practice Nurse room duties including wound care, compression bandage, cervical smears, baby and travel immunisations, and pessary ring procedures as well as experience in Chronic Disease Management.
We hold a PMS Contract in South Gloucestershire.
We use EMIS Web and Ardens.
We provide a full range of enhanced services.

We have an active working relationship with our PCN member practices.
Main duties of the job We are looking for an experienced nurse that has held a practice nurse position in a General Practice.
It's a busy environment, so you will need to be able to work under your own initiative as well as part of a successful team.
Additionally, you will need to be flexible and amenable to the changing demands of primary care.
About us St Mary Street is a well-established Practice located in the semi-rural town of Thornbury near to the Severn Estuary.
We are a dedicated team of 30 people (directly employed) who are supported by PCN employed colleagues including a Pharmacy team, First Contact Physiotherapists, a Mental Health nurse, and Social Prescribers.
We are a friendly, successful 4 Partner Practice committed to providing high-quality patient health and care services to our 8,704 patients and growing.
Our cultural ethos is built upon core values including safe, accessible, and continuous care for each of our patients.
We are also a GP training practice with 3 of our 4 Partners registered as trainers.
Therefore, we commonly have at least one learner with us at any given time.
In addition, we are supported by three part-time Salaried GPs, two Advanced Nurse Practitioners, a highly successful nursing team, and a dedicated and supportive administrative and reception team.
We are a social bunch and fare well in the annual GP Patient Survey.
Our staff turnover is low, and the team supports and looks out for each other especially as patient demand on our services reaches an all-time high.
Job responsibilities The following are the core responsibilities of the Practice nurse WITH Chronic Disease Management.
There may be on occasion a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels:
Implement and embed health promotion and well-being programmes. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients' health. Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF. Provide wound care (ulcer/Doppler etc.)
to patients. Provide immunisations (childhood, seasonal, etc.
). Provide Cervical Cytology screening. Request and process pathology results. Maintain accurate clinical records in conjunction with extant legislation. Ensure accurate use of SNOMED codes to support QOF and LES/DES targets. Chaperone patients where necessary. Assist GPs with minor surgery when required. Prioritise health issues and intervene appropriately. Support the team in dealing with clinical emergencies. Implement vaccination programmes for adults and children. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice). Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.
). Support the clinical team with all safeguarding matters, in accordance with local and national policies. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately. Person Specification Experience Experience of working in a GP Practice.
