We are an independent, registered charity providing front-line support to young people and people experiencing homelessness to open pathways out of poverty and homelessness - breaking the cycle of disadvantage.
Our focus is on respect, client-centred care, and harm prevention so people can lead longer lives and have improved health, meaningful employment, and a greater sense of belonging, safety, and security through our holistic model of care.

A bit about the role
The Practice Nurse will play a pivotal role in delivering compassionate, client-centred care to Melbourne's most vulnerable populations through a collaborative, multidisciplinary approach.
This role focuses on providing culturally appropriate, holistic support to individuals with complex healthcare needs, ensuring their health, functional ability, and quality of life are prioritised.
Key responsibilities include triaging clients, conducting clinical assessments, screenings, consultations, care planning, and follow-ups.
The Practice Nurse will coordinate appropriate referrals and work closely with clients, carers, and a multidisciplinary team of healthcare professionals to achieve personalised health goals and outcomes.
Operating within the Nursing and Midwifery Board of Australia's national competency standards, the Practice Nurse will uphold best practice principles, ensuring all patient services are delivered ethically and effectively.
The role also includes maintaining regulatory compliance, managing stock and equipment, and accurately charging for services per established protocols.
This position offers an opportunity to make a meaningful difference by addressing the complex health needs of vulnerable individuals, leveraging a team-based, multidisciplinary approach to deliver comprehensive, high-quality care.
Role Requirements:
Experience working with in Community, AOD or Homelessness sector
Minimum 2 years' experience in a similar role
Registered Nurse with current registration with Health Practitioner Regulation Agency (AHPRA).
Demonstrated understanding of the professional, ethical and legal requirements mandated by the relevant Regulatory body
Evidence of Continuing Professional Development
Experience and/or demonstrated commitment to working with homeless and marginalised populations experiencing psychiatric, medical and substance use conditions within a harm reduction model of care
