APS4 equivalent 43+ p/h + super Located in Bundaberg, Queensland Up to 18 months with potential to extend, pending business and performance requirements. About the Team Join our dedicated team at the Younger People in Residential Aged Care team, a vital part of the Hospital Interface Branch. We specialise in providing support for individuals with disabilities who are living in or at risk of entering aged care facilities prematurely. Our mission is to implement the government's Strategy, focusing on participants under 65 years old. About the Role As an APS4 Planner, you will play a crucial role within our team, operating within defined parameters to support the objectives of the organisation. You will engage with stakeholders, gather information from participants, and make funding decisions to ensure high-quality planning and service delivery. This role offers the opportunity to contribute to resolving complex enquiries, liaising with key stakeholders, and meeting planning targets. Gather information from participants and planning partners to facilitate evidence-based decision-making. Utilise policies, practice guidance, and legislation to make funding decisions aligned with participant needs. Work closely with participants and representatives to identify current and future supports. Liaise with stakeholders, including Partners in the Community, to deliver services effectively. Resolve issues and complaints related to participant plans. Complete administrative and data entry tasks accurately. Contribute to achieving key performance indicators and planning targets. About you To thrive in this role, you need: Strong verbal and written communication skills. Experience engaging with multiple stakeholders. Knowledge of the disability, health, or aged care sectors. Experience gathering information from participants and Health over the phone. Ability to make collaborative access decisions using policies and legislation. Skill in identifying information required for collaborative access. Experience liaising with Health Liaison officers and other key stakeholders. Commitment to meeting Disability Reform minister commitments and agency SLAs. Adaptability to thrive in a changing environment. Be able to pass and hold a national police check and worker screening check. Must be an Australian Citizen.
