Reporting to the TCP Service Manager, your key responsibilities will include assisting patients with accessing the benefits and services needed to support their hospitalisation, discharge, and ongoing care. You will work closely with resource providers, negotiating and coordinating the necessary services to ensure a smooth transition for each patient.
In addition, you will be the primary contact for managing relationships with aged care facilities, maintaining up-to-date knowledge of available options, and liaising with both facilities and families. You will also provide vital support to clinical and multidisciplinary teams in the placement of patients into appropriate residential or community care facilities.
