About the role: Perform comprehensive assessments to evaluate clients' abilities, limitations, and specific needs.
Create tailored rehabilitation plans with achievable goals and targeted interventions.
Identify psychological barriers to returning to work (RTW).
Provide emotional and mental health support, assisting clients in managing their conditions.
Collaborate with healthcare providers, employers, and stakeholders to support rehabilitation efforts.
Help clients explore alternative career paths and offer guidance on job placement and vocational training.
Inform clients and families about the rehabilitation journey while advocating for clients' rights.
Track client progress, modify plans as required, and maintain accurate records and documentation.
Deliver crisis intervention and connect clients with appropriate emergency services when necessary.
