The Wide Bay Hospital and Health Service is seeking a skilled and compassionate Senior Physiotherapist to join our Geriatric Evaluation Management / Rehabilitation Hospital in the Home (GEM/GEMR HITH) service. This innovative program focuses on improving the health outcomes of older patients in their own homes, reducing unnecessary hospital admissions, and supporting a quicker recovery through tailored care. This service is integral in managing frailty, helping to avoid hospital admissions and reduce length of stay for patients with complex, age-related conditions.
Your opportunity: To deliver high level Physiotherapy clinical services and professional expertise for GEM / GEMR HITH patients within the Integrated Care Services, Fraser Coast.Work autonomously within the interdisciplinary Integrated Care Rehabilitation Team to manage, coordinate and provide optimal, responsive and flexible Physiotherapy services for GEM/GEMR patients within the GEM /GEMR HITH clinic and community settings.The Geriatric Evaluation Management / Rehabilitation Hospital In The Home (GEM/GEMR HITH) is an initiative of the Wide Bay Hospital and Health Service funded through the Queensland Health Improvement Unit. The service is focused on improving health outcomes for patients over the age of 65 years or 50 years for Aboriginal and Torres Strait Islander people, supporting avoidable admission and reducing length of stay in hospital for patients with age-related physical, functional and/or cognitive decline associated with frailty.Our offer: You will receive a competitive remuneration package value up to $122,220 p.a., comprising salary rates $64.64 - $69.56 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4) (Permanent Part Time, 60.8 hours p.f.)
