An exciting opportunity is currently available for an experienced Allied Health Case Manager to join the Integrated Care Rehabilitation Services Team at the Hervey Bay and Maryborough Hospitals within the Wide Bay Hospital and Health Service.
The Wide Bay area is arguably one of the most beautiful regions of Queensland.
Your opportunity:
The Transition Care Program (TCP) is an exciting jointly funded initiative of the Commonwealth Department of Health and Ageing and Queensland Health, offering time-limited and goal-oriented flexible rehabilitation care packages to older persons who have had a recent acute or sub-acute hospital admission and live within the Wide Bay Health Service region.
The position of Allied Health Case Manager will have responsibility for planning the provision of care, coordinating and monitoring rehabilitation of allocated clients within the Transition Care Program.
The Case Manager practices autonomously as part of the multidisciplinary team, applies critical thinking, and provides leadership in clinical decision making regarding the rehabilitation of Transition Care clients.
Temporary Full Time (76 hours p.f.)
from date of appointment until 31st July 2025, with the possibility of extension.
