The Transition Care Program (TCP) is an exciting jointly funded initiative of the Commonwealth Department of Health and Ageing and Queensland Health, offering time-limited and goal-oriented flexible rehabilitation care packages to older persons who have had a recent acute or sub-acute hospital admission and live within the Wide Bay Health Service region.
It is aimed at older clients, but age alone is not an eligibility criteria.The position of Allied Health Case Manager will have responsibility for planning the provision of care, coordinating and monitoring rehabilitation of allocated clients within the Transition Care Program.
As well as providing holistic case management of clients, this position will provide high-level Allied Health treatment in their area of Allied Health expertise to assist in the rehabilitation of TCP clients.
This treatment will be provided to any client on TCP where indicated, not only the clients who are being case managed by this person.The Case Manager practices autonomously as part of the multidisciplinary team, applies critical thinking and provides leadership in clinical decision making regarding the rehabilitation of Transition Care clients.Temporary Full Time (76 hours p.f.)
from date of appointment until 31st July 2025, with the possibility of extension.
Our offer:You will receive a competitive salary between $4,912.70 - $5,286.40 p.f., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4) (Temporary Full Time, 76 hours p.f., from date of appointment until 31st July 2025, with the possibility of extension).
