HammondCare is forming an exciting new internal Allied Health Care Team to work in Residential Care. You will have the autonomy to work with residents to achieve the best care outcomes while being part of a broader team of in-house Allied Health professionals supporting frontline teams.
Together, you will deliver resident-centred, evidence-based care in an environment that promotes continuous quality improvement consistent with the values and ethics of HammondCare and the Australian Physiotherapy Association.
Essence of the role Provision of timely, effective and high-quality physiotherapy service and advice to residents and staff in residential care. Collaborate closely with residents, their representatives, staff, and the broader Allied Health Team to assess functional care needs, set care goals, and provide treatment and interventions to deliver the best possible outcomes for residents. Participate as a member of the HammondCare team in the application of the Mission and values to the pursuit of its objectives in a person-centred, continuous quality improvement work environment. Participate in quality improvement projects and continuing professional development. About you Commitment to engage and align with HammondCare's Christian Mission, Motivation and Mission in Action. A tertiary qualification in Physiotherapy including current AHPRA registration. Practical experience or sound knowledge in Aged Care. A current driver's license, safe driving record and access to your own car, with a willingness to travel between HammondCare's sites and services. Effective communication and interpersonal skills (written and verbal). Demonstrated organisation and time management skills. Demonstrated ability to work independently and as part of a multidisciplinary team.
